The concept of emphasizing, discussing, and training on giving prehospital death notification is, unfortunately for medics and our patient's families, a relatively new concept. Additionally, newer evidence shows that an increased frequency of fatality exposure increases EMS burnout rates. So...everyone wins by an increased focus on improving our skills and comfort with telling families that their loved ones have died. Within EMS, significant time is spent learning high risk, low frequency procedures and the pharmacology and physiology surround these events. Like the rest of the country and world, our Montgomery County hospital partners and ED's have been under severe strain due to a myriad of factors related to the COVID-19 pandemic. This had led to a significant increase in EMS wall times which, in turn, places that severe strain back on the prehospital system. There has been significant development and change surrounding which patients are best served by immediate revascularization post-ROSC over the past 12-18 months. On that note, we discussed the COACT study previously on the podcast, but uncertainty still exists. OHCA and post-ROSC patients are wildly heterogenous and one size will never fit all. We have all been guilty of caring for labeling and judging “pseudoseizures.” First of all, this term is outdated, and this episode looks at a study examining the mortality in psychogenic seizure patients. Electrical storm has dismal outcomes. It is most frustrating because we know the cause is very likely coronary occlusion (~75%)! Sometimes in medicine, we do things just because they’ve always been done that way, which is terrible reasoning. Valsalva maneuver followed by the two-syringe technique for adenosine administration has been standard practice in PSVT management for decades. Join Dr. Dickson and our esteemed colleagues from across the globe to hear how their systems have responded to the COVID-19 pandemic and what they have learned from the experience thus far. Dr. Colin Graham is an EM specialist from Hong Kong who practiced there for the first SARS outbreak and is now tackling Corona Part 2. Additionally, we’ll be joined by Dr. Al Meyer, an EM specialist in Melbourne, Australia. This episode truly gives us a worldwide COVID-19 perspective. Colin Graham qualified from the University of Glasgow in 1994 and completed basic surgical training and then specialist training in emergency medicine in Scotland. He came to the Accident and Emergency Medicine Academic Unit at the Chinese University of Hong Kong in 2004; he was promoted to Professor in 2007 and was appointed Director in 2015. He has broad research interests within emergency medicine and has published widely. Dr. Graham is an active clinical teacher and he co-authors the Oxford Handbook of Emergency Medicine. He is also currently Editor-in-Chief of the European Journal of Emergency Medicine (since 2009) and has served on the Executive and Council of the European Society for Emergency Medicine. Dr. Graham was elected to the Council of the Hong Kong College of Emergency Medicine in 2013 and 2016 and has served as Dean of Students at Morningside College in CUHK since 2016. Dr. Alastair Meyer completed his specialist training in emergency medicine in Scotland and has worked in academic emergency medicine in Melbourne Australia. He is currently the emergency department director at Casey Hospital in Melbourne. He has numerous peer reviewed publications and has lectured both with AU and internationally. Dr. Meyer is an avid cyclist and word traveler in his spare time.

We’ve discussed TXA, both for trauma and for epistaxis, on the podcast previously. Medics often ask about TXA in the acute GI bleed patients who are in hemorrhagic shock...“What can it hurt? We all love cardiac arrest with VF as the presenting rhythm. These are the ones that walk out of the hospital. But, what happens when you shock, shock, shock and still VF? Today the podcast team addresses refractory ventricular fibrillation/electrical storm and specifically the role of esmolol in this condition. Esmolol is an emerging treatment for refractory VF that makes absolute physiologic sense, however the data is limited. One of the most difficult situations in prehospital care is the high-risk patient refusal of EMS transport. This topic is far from straight forward for both learners and teachers alike. Lowering tidal volumes in effort to reduce lung injury following initiation of mechanical ventilation is far from a new idea, the original ARDS-NET data are nearly 20 years old. Dr. Jerry Snow, a medical toxicologist and emergency medicine physician from Phoenix joins the show for a fact-filled and often hilarious talk on prehospital management of snakebites. In the past 24 months, stroke care has evolved in the way we evaluate strokes in the pre-hospital space and how they are worked up and treated in the hospital. Large vessel Occlusion (LVO) stoke is a large caliber vessel clot causing stroke symptoms. New techniques and devices in endovascular retrieval of these clots along with intravenous Tpa has shown great success in five randomized trials published in 2015.(1) The number we need to treat for one improved neurologic outcome at 90 days according to data from these trials is 2.6 patients. This is a massive breakthrough in the way we approach acute stroke care. Improved outcomes in these trials are directly associated with times to reperfusion therapy and EMS must change the way we diagnose and rank stroke patients in order to get them an endovascular capable facility when appropriate. In the first podcast our team will review all of the pertinent data from these trials and suggest how EMS should approach this LVO stroke diagnosis. In the second part of our stroke awareness series we talk with Dr. Peter Antevy, an EMS medical director from Florida who is an expert of developing regional systems of care for these LVO stroke patients. He will discuss development of their system and exciting new data that demonstrates a treatment benefit up to 24 hours after onset of symptoms in some subsets of these stroke patients. Anaphylaxis is a common life threatening disease that prehospital providers encounter on a regular basis. These patients are in need of early treatment with Epinephrine.
