As one person's negligence is looked at as potentially causing future risk of life ending consequences and the failure of the mission.  As in the Family Court, CPS and the divorce sector, individuals who "screw up," can use mental illness, Parental Alienation, abuse and even the fatalities of children and generally end up getting promoted and or get very little to no negative attention at all.
