Here is what we will cover in this show:

1) Let's discuss abutment selection for screw retained implant crowns.  Zero Bone Loss Concepts discusses using titanium bases vs. using customized abutments and selecting the titanium base cuff height to allow customization of the emergence.  One reason is the OEM fit.  Another reason is to have polished zirconia against the tissue rather than titanium.  A downside is that retention of SOME of the bases is more limited.  What are your thoughts on this?

2) For multiple unit prosthetics that are screw retained, how do you feel about using non-engaging titanium bases vs. using multi-unit abutments and a secondary screw?

3) How important do you think subgingival material selection is?  Linkevicius builds a case that polished zirconia is biologically the best and that we should avoid veneering subgingivally and avoid metal subgingivally where possible.  That makes shade more challenging for certain cases, especially with longer teeth where you might want to add either pink porcelain or root-colored porcelain.  It also makes it tougher if you like to use PFM with some lab technicians who prefer it for challenging shade matching.  Does this change your mind or were you already there?

Clensability vs. Esthetics vs. Lip Support- often these things are at odds in the edentulous maxilla when it comes to implants.  There just has to be a better way!  Darin Dichter of Spear Education is our go-to guy on this topic.  He knows a great deal about unsplinted full arch implant restorations.  We began this conversation in our last interview with Darin and we get into the nuts and bolts of actually pulling this difficult but extremely effective restoration off.  Can we finally get the lip support we want without a giant lettuce-trapping anterior cantilever?

Dentistry as a whole is struggling with producing quality impressions…but just how bad is it?  A recent JADA article answers that question and we take a stab at some possible fixes.  Also, an endo article we stumbled upon a couple of months ago ends up being a real revelation as to how long we can really wait before putting crowns on endodontically treated teeth before failure rates go up.  How important is coronal seal?  Finally, we talk about how digital impressioning changed our retraction techniques and product recommendations.

Over the last year, we changed the way we look at wear and occlusal problems through a better understanding of how occlusion really works and how sleep problems may be causing some of our TMD patients to get worse!  We also learned about the most predictable ways to restore patients with challenging occlusal problems and it transformed the way we treatment plan- again!  Join us as we recap our journey over the last year as we continue to try to take it to the next level!

Our series on endo continues as we discuss how to choose an endo file system.  We start with the history of endo files and different generations to show how advances in technology have changed techniques.  Then, we spend time discussing how to decide between all of the various systems out there- what features matter and which don’t.  And, finally, we give direct recommendations about what we think you might want to buy!

The Dental Guys get a chance this week to interview someone quite interesting: Dr. Sameer Puri, one of the founders of cerecdoctors.com as well as an educator at Spear Education. We got to record this one in the Spear studio and really got to know what makes Sameer excited about CEREC!  We talk about what CEREC can and can’t do, how it compares to the lab, and why he feels that CEREC is more than just a business decision but instead a passion that he wants to share with fellow doctors.  He challenged us to think differently about how we make technology decisions in our practices and also how chairside milling could change the way you do dentistry.  We asked him to make us believers in CEREC, and you’ll have to listen to see what came of that!

Our series on endo continues!  After our last two episodes, we are ready to get working length and start irrigation.  We will talk through the science and clinical techniques that help make endo more predictable and we will discuss some of the controversies such as lubricants, which irrigants to use, and many other important topics.
